The American Academy of Pediatric Dentistry advises that a child's first dental visit should happen by the first birthday, or within about six months of the first tooth appearing. Most Indian parents hear this and are surprised, since there is nothing visibly wrong. The purpose of that visit is not treatment. It is to check how the teeth are coming in, show you how to clean them, and talk through feeding habits.

Feeding habits cause more decay in small children than anything else. A baby who falls asleep with a bottle of milk, or a toddler who is soothed with a sweetened biscuit or a spoon of sugar-laced dal-bhaat at bedtime, bathes the teeth in sugar through the night. Brushing should begin when the first tooth appears. School age and teenagers: six to eighteen

Around the age of six the first permanent molars arrive, behind the last baby tooth, and many parents mistake them for baby teeth that will fall out. They will not, and they are the teeth most prone to early decay because their deep grooves trap food. A thin protective coating called a fissure sealant, applied in a short painless visit, is one of the most effective ways to protect them.

This is also the age of tiffin boxes, chocolates, chikki, packaged namkeen and cold drinks, and the age at which cavities appear in bunches. Frequency matters more than quantity. A child who nibbles sweets through the afternoon is at greater risk than one who eats a larger portion at a single sitting and then brushes. Rinsing with water after snacks and brushing at night are the habits that count.

Alignment is the other question of these years. Many orthodontic problems can be assessed from about age seven, though braces usually begin between roughly eleven and fourteen, once most permanent teeth are in. Some children benefit from early guidance of jaw growth, while others simply need monitoring. A check with a dentist tells you which group your child belongs to. Adults in their twenties to fifties

This is the group that skips check-ups most often, because nothing hurts until it hurts a great deal. Adults are also the group most affected by stress-related habits. Many people clench or grind their teeth at night without knowing, and they present with flattened tooth surfaces, sensitivity, a sore jaw in the morning or small cracks. Gum health becomes the central issue from the thirties onwards. Bleeding when you brush is not normal and is not a sign that you are brushing too hard; it is inflammation, and left alone it can progress to bone loss. Pregnancy deserves its own mention, since many women postpone dental care for nine months out of caution. Hormonal changes make gums swell and bleed more easily, so a cleaning in the early months is helpful rather than harmful. Pain or infection should be treated promptly. Non-urgent work is usually done in the second trimester, when lying back in the chair is most comfortable, and you should tell your dentist you are pregnant before any X-ray or prescription is considered.

Parents and grandparents: sixty and beyond

Older patients bring a different set of problems. Many take several medicines a day, and a surprising number of these, including some for blood pressure, depression and allergies, reduce saliva. A dry mouth raises the risk of decay at the neck of the tooth, where the gum has receded, and makes dentures harder to wear. Sipping water through the day and discussing the medicines with the dentist, not stopping them, is the right approach.

Tooth loss is common, and the question is usually how to replace what is missing. The options run from a removable partial or complete denture, to a fixed bridge, to implants, and the right choice depends on bone, health, budget and what the patient can manage. Not everyone is a good candidate for implants, and a well-made denture is a perfectly respectable answer for many people. And if your household already has a dentist you trust and who knows your history, there is no reason to move for the sake of having everyone in one place.
